Extra Pages

Global Visa Services can have the Passport Office sew extra pages into your existing passport booklet. The US State Department considers this procedure a passport amendment and uses the DS-4085 passport amendment form.
  1. Application : The current form for adding pages to a passport is DS-4085. The form is available directly from the State Department at the link below.

  2. Passport : The extra pages will be put inside so include it.

  3. LOA Documents : Signed and completed LOA document. Please note that signatures must be original, facsimiles or copies are not accepted by the Passport Office.

  4. Itinerary : Include a copy of your tickets or flight schedule. Please note that the State Department will only rush the issuance passports if the itinerary demonstrates immanent departure. Applications with departure dates two weeks into the future will not be accepted at the Passport Office. If additional visa work is required than an exception can be made.

  5. Payment: Include two separate payments. One check payable to US Dept. of State for $60.00, and a second check payable to Global Visa Services for $175.00 + return shipping. Please reference the passport orderform for shipping fees.

Global Visa can usually have your passport request issued within a twenty four hour period if your itinerary demonstrates a scheduled departure with in a weeks time. The US State Department reserves the right to take longer without reason, require additional documentation, or request the applicant appear in person at the National Passport Center. These cases are rare but applicants should be aware that delays can occur on occasion. Global Visa is not responsible for any delays in your travel in such cases. Please also note that the US Department of State can put into suspense any application from individual with outstanding public debts such as taxes to the IRS or delinquent child support payments. In those cases it is up the individual to make payment to the appropriate jurisdiction where monies are due before passports can be issued.
