Replacement for Lost or Stolen Passport

A lost or stolen passport can be replaced. Although a serious event, passports are lost and the State Department has a system for their reissue. Your identity and citizenship must be proved as condition for passport reissue. You must apply as if you were getting a first passport. This means the applicant must prove their identity to a passport processing postoffice or an authorized county clerk's office.

Lost or Solen Proceedure Checklist

  1. Form DS-64: Sign and complete form DS-64. Include with DS-11 sealed package, see below.

  2. Form DS-11: This is the application for a new passport. Please follow the instructions regarding the proper execution of a DS-11 sealed document. All instructions pertaining to forms, fees and proceedures for a new passport are applicable to the replacement of a lost or stollen passport.

  3. Extra Identification : It will assist your application to include copies of additional ids. Items such as copied driver licensces or another government issued photo id will fascilatate your request for a new passport.

Lost/Stolen Passport Notes

Report Passsport Stolen : To protetct your personal identity and defend against fraudulent entry into the United States, the State Department urges that lost or stolen passports be reported as such immediately. The completion of form DS-64 is considered sufficient notice for most cases.
US citizens traveling overseas can report their passport lost or stolen at the nearest US Embassy or Consulate.

Multiple Lost Passports : Please note that multiple requests for lost passports is a suspicious activity that will at the very least slow down your request as the US State Department scrutinizes your file for legitimacy. If your case proves to be valid it will be processed. The US State Department reserves the right to request a personal interview with any applicant. It is our experience that several lost or stolen passport requests warrants the applicant to some sort of personal verification with the State Department. This can be as simple as a personal phone call with an official or as demanding as a personal interview at the National Passport Center in the District of Columbia. In these cases of government inquiry, Global Visa Services can not be held responsible for any delays in your travel.

Global Visa can usually have your passport request issued within a twenty four hour period if your itinerary demonstrates a scheduled departure with in a weeks time. The US State Department reserves the right to take longer without reason, require additional documentation, or request the applicant appear in person at the National Passport Center. These cases are rare but applicants should be aware that delays can occur on occasion. Global Visa is not responsible for any delays in your travel in such cases. Please also note that the US Department of State can put into suspense any application from individual with outstanding public debts such as taxes to the IRS or delinquent child support payments. In those cases it is up the individual to make payment to the appropriate jurisdiction where monies are due before passports can be issued.
